In the Older People & Hospital Social Worker role, you will:
- Provide high-quality assessments, support planning, and interventions for older people and adults being discharged from hospital settings
- Work closely with multidisciplinary teams, families, and external agencies to ensure safe and effective care pathways
- Contribute to safeguarding and promoting the wellbeing and independence of service users
- Manage a varied caseload in line with statutory requirements and departmental procedures
To be successful in the Older People & Hospital Social Worker, you will need:
- A recognised Social Work qualification and a minimum of 3 years’ post-qualifying experience
- Registration with Social Care Wales
- Strong IT skills, alongside knowledge of relevant systems and software
- A solid background in hospital social work and working with older people
- Excellent interpersonal, communication, and teamwork skills
This is a full time role working hours are Monday to Friday, 37 hours per week. You’ll start this temporary position for initially 12 weeks, however it does have the potential to be extended. A competitive hourly rate is on offer of £28.01, plus overtime and other benefits.
You’ll be working from offices in Colwyn Bay and there is an expectation that you live locally or with in a commutable distance as the position is located in Conwy County.
