We are currently recruiting for an experienced Social Worker to join an Older People & Hospital Social Work Service on a temporary basis. In this role you will be responsible for providing a high-quality social work service to vulnerable adults and those requiring additional support.
In the Social Worker role, you will be:
- Provide high-quality social work support to vulnerable adults and older people.
- Work closely with citizens, families and carers to assess needs and identify appropriate support.
- Promote independence, resilience and positive outcomes.
- Ensure individuals and their families are at the centre of decision-making.
- Work collaboratively with colleagues and other professionals.
- Maintain accurate records and case information.
- Use professional knowledge and judgement to support positive outcomes.
- Communicate effectively with citizens, families, carers and partner agencies.
To be considered for the Social Worker role, you will need:
- A recognised Social Work qualification.
- A minimum of 3 years' post-qualification experience.
- Current registration with Social Care Wales.
- Previous experience working with older people, ideally within a hospital or adult social work environment.
- Excellent teamwork and interpersonal skills.
- Strong communication and relationship-building skills.
- Good IT and software skills.
- A person-centred approach and a commitment to promoting independence and wellbeing.
This is a 37-hour-per-week position, working Monday to Friday and based in the Abergele area, with a pay rate of £28.01 per hour. Candidates should live locally or within a reasonable commuting distance. The assignment is expected to last approximately 12 weeks, with potential to extend.
